Another suggestion is Approval Manager (http://www.metacommunications.com/products/approval_manager/). This might be more suited for larger design firms but it offers online proofing and review along with advanced workflow management. It can also be integrated with other project management modules for a more advanced system.

If any feedback service is still needed, you could try http://idea.informer.com/. It's easily customizable and what's even more important completely free.

I would suggest another annotation tool: UserThought.com feedback solution.

It provides a feedback form with content highlighting feature and supports feedback preview directly on your website as overlays.

The solution is customizable, cross-browser, easy to integrate (1 line of HTML code) and
DOES NOT REQUIRES ANY WEB BROWSER PLUGINS.

So, any your visitor could use it to highlight and comment on some fragments of your website!

Try it for free (30 days free access to all advanced features)!

An interesting up-and-comer: <a href="http://www.yaytrail.com" rel="nofollow">http://www.yaytrail.com</a>

Different from others reviewed above in that it allows for in-line annotation of pages. No sticky notes or side-bars or content that's layered on top of the original page. It's a bit more like Google Wave style interaction. First annotation tool I've seen that diverges from the sidebar/sticky-notes model, which is kind of crazy when you think about it! Annotation's been around a long time, and this is the first time in a long time I've see anyone try to do something different with the fundamental interactions involved.

It's at quite a raw stage, but it's something new, with a simpler/lighter presence than other tools I've found (because of the lack of sidebars, sticky notes and so on).
